When describing your problem please try to include the URL (web address) of the form page you are using.
For filings, be sure to include the following information with your request:
- your name, company/office and return email
- commodity description and existing commodity number if known
- effective date
- expiration date if any
- origin port / point
- destination port / point
- rate basis - FCL / LCL
- container constraints if any
- carrier restrictions if any
- control numbers if any
